High-quality primary care is foundational to providing “whole person care” that emphasizes prevention, as well as team-based and relationship-based care. To help improve primary care, the Center for Medicare and Medicaid Innovation (CMMI) has developed and tested several multi-payer advanced primary care models over the last several years. Earlier this year, CMMI announced another opportunity for providers and payers to participate in an innovative primary care model. The new Making Care Primary (MCP) model seeks to support the delivery of high-quality, advanced primary care services, prioritize meeting both the medical and nonmedical needs of patients, advance health equity, and address fragmentation throughout the health care system. CMMI is accepting applications for the MCP model through November 30, 2023, with the new model slated to launch on July 1, 2024.
